Parental Photo Consent Form

We recognize the need to ensure the welfare and safety of all young people taking part in any activity associated with the team.

In accordance with our child protection policy we will not permit photographs, video or other images of young people to be taken without the consent of the parents/caregivers and children. As your child will be taking part in team related events (practices, games, tournaments, team photos, etc.), we would like to ask for your consent to take pictures/videos of the event or activity that may contain images of your child throughout this season. It is likely that these images may be used:

·      As a record of the activity or the event

·      To be placed in the photographer’s portfolio, Facebook page, or website.

·      Publicity material for further activities or events on leaflets/websites/magazines/flyers

·      For season photos.

We will take all steps to ensure these images are used solely for the purposes they are intended. If you become aware that these images are being used inappropriately you should inform us immediately.

*All photos are free to share on social media. We ask that you do not modify (crop or add filters) or reproduce the images as they are purposely developed in low resolutions for the purpose of sharing on social media and reproducing the images will not come out in its best quality.
